Incidents of Personal Injury or Third Party Property Damage

Immediately advise details to the Insurance Manager at the NSW Synod Office. If necessary, an accident report form will be dispatched for completion by Congregation Officers.

PLEASE NOTE:

The form must be completed by Church Officers and the injured party is not to assist in completing this form nor sign it.

UNDER NO CIRCUMSTANCES is liability for any injury or damage to be admitted.

To do so may deny the Church the benefit of cover provided by this policy.
